Scuba & Tankless Diving Courses & Experiences
Required for all scuba diving courses, guided dives, and underwater experiences
Liability Release and Assumption of Risk Agreement
Required for all scuba diving courses and guided dive experiences. This document acknowledges the inherent risks of scuba diving and releases Gold Coast Scuba from liability.

Standard Safe Diving Practices – Statement of Understanding
Confirms that you understand and agree to follow PADI's standard safe diving practices throughout your course or dive experience.

Medical Statement
A health screening form required before any in-water training. Please note: if you answer YES to any questions, your physician must complete the remainder of the form for medical clearance before you can participate.

Youth Diving Responsibility and Risks Acknowledgement
Required for all divers under 18 years of age. A parent or legal guardian must review and sign this form acknowledging the responsibilities and risks associated with youth scuba diving.

Required for snorkeling tours, free diving, and Seabike/scooter excursions
Snorkeling & Free Diving Waiver
Required for all snorkeling tours, free diving experiences, and underwater scooter (Seabike) excursions. Acknowledges the risks associated with surface water activities.

Snorkeling Safe Practices Statement of Understanding
Confirms your understanding of safe snorkeling practices, buddy procedures, and surface signaling protocols used during Gold Coast Scuba tours.

Florida Addendum – Notice To The Minor or Guardian Form
Required for participants under 18 joining snorkeling or free diving activities. Florida law requires this additional notice to be signed by a parent or legal guardian.

Equipment & Gear Rentals
Required when renting any scuba or snorkeling gear from our shop
Equipment & Gear Rental Waiver
Required when renting any scuba or snorkeling equipment from Gold Coast Scuba. Covers tanks, BCDs, regulators, wetsuits, masks, fins, and all other rental gear.

All waivers must be completed before your scheduled activity. We recommend completing them at least 24 hours in advance.
Medical Statement: If you answer YES to any medical question, your physician must complete the remainder of the form. This may delay your participation if not submitted in advance.
Minors (under 18): A parent or legal guardian must be present to sign all youth-specific forms. Florida law requires the Addendum form for all minors.
